Severe h pylori for 1 year, failure to eradicate

Hey, I am seeking advice about my h pylori that I have had for over a year. Symptoms first started appearing in march 2020 and I had been constantly throwing up for 2 weeks. I am active in the gym so this was a wrecking ball to me as it caused me to lose 10kg. The throwing up was terrible and whenever I tried to eat, my heart would beat rapidly and I would start exhibiting symptoms that seemed like someone with anxiety would have, every time I took a bite of something, and then i would throw it up. I decided to finally see a GP where I got given a breath test and got diagnosed with helicobacter pylori.

I'd like to note if it may assist that it seemed like an absurd amount of the bacteria, on the sheet it said >500 indicates positive result of helicobacter, and it showed that I had a number of in the 3000s.

After being put on triple therapy for 14 days (nexium, amoxcillyn and klacid) I felt better but there was something still off about my stomach. Idiotically I decided there's no point of getting retested.

About 6 months later, I began throwing up again and felt the exact same symptoms I had previously felt. I saw the doctor and got retested and came back positive but this time with a reduced amount of helicobacter (it went from 3000 to about 2000). I got put on the same exact triple therapy again.

After I finished the triple therapy, I waited the one month and decided not to be an idiot and get retested, which again, came back positive this time being in the 1000s. The doctor put me on another course of triple therapy.

After the third course of triple therapy with the same medication, I have gotten retested and it just came out that I have it again. My GP visit is next week where I think he'll send me off to a specialist.

Yesterday I threw up and can no longer seem to eat as much as I used to. This is really impacting my passion as well due to not being able to sustain a proper diet that requires a surplus of calories.

If anyone has any advice on how I can go about getting adequate nutrition in whilst I have this infection that would be appreciated. Any advice about eradication would be immensely useful as well.

briefly,

1. Vomit is not a common symptoms of H. pylori infection. You probably have other underlying problem. H. pylori may have triggered it.
2. The diagnostic result of 3000, although it's related to the number of bacteria you have, it is not related to the severity. You can use that data to tell how accurate the test is. If the cut off is 500, and you got 1000 and above, that's pretty definite that you have H. pylori. If you have a reading of 550, then you'll be asking whether that reading is trustworthy.
3. Your doctor should know about not getting the same treatment after the failure of first treatment. Your H. pylori is obviously resistant to the antibiotics, therefore there is no point taking the same drug over and over again.
4. Since you did feel better after treatment, I think you should continue to seek medical treatment to get rid of H. pylori.

By my description, would you know what could be causing the vomiting or what may have been triggered by the hpylori? Also, being that it has been a year do you think I am at high risk to development of ulcers or even gastric cancer? I took this upon my doctor and he said an endoscopy isn't necessary as I am only 18, do you think this is correct?

he is right. 18 is very young. Usually young people like you have very healthy acid secretion and therefore you are more prone to get stomach ulcer.

I dont know what causes your vomit. But you can try focus on how to get rid of your resistant H. pylori.

Here is the tip.
1. use high dose PPI. eg. Rabeprazole 20mg TID.
2. use bismuth 220 mg QID and amoxicillin 1 g TID. H. pylori almost never become resistant to these antibiotics.
3. use one more antibiotic that you have not tried before (except tetracycline, as it doesnt work very well with amoxicillin).
