Hello,

My stomach problems started in the 3rd month of this year, when I started to develop various symptoms: stomach cramps, stomach ache, nausea, burning sensation and bloating in the upper abdomen area. Since the symptoms started during the lockdown period due to the coronavirus pandemic, I could not go to the doctor. I've eliminated processed foods from my diet, I've stopped drinking coffee and alcohol. Things were starting to get better, but without any major progress. I finally decided to go to a specialist and in July I did an endoscopy. Specialist did not take sample for the biopsy, he just told me that I have chronic antral gastritis due to helicobacter pylori. Endoscopy finding also states: reactive bulbopathie and gastroptosis. He prescribed to me, since I have penicillin alergy, clarithromycin 500, metronidazole 500 mg both twice a Day for 7 days, Pantoprazole 20 mg x2 for 2 months, and he told me if I find bismuth subcitrate 120mg 4x, to take it during 21 days (I managed to find bismuth on the second day of therapy).

I think I have used metronidazole and clarithromycin for other conditions in the past before this specific therapy. Is 7 days of therapy with the addition of bismuth sufficient if there is dual resistance to antibiotics? My symptoms approved a little bit, but still have stomach discomfort, fullness after meals, sometimes morning nausea and burning sensations. I noticed, after I finished therapy, that thick white coating on my tongue dissapeared, and also long lasting skin symptoms on the face and scalp, pruritus, condition similar to seborrheic dermatitis vanished. 2 months have passed since finishing the antibiotics, a month and a half since bismuth and one month since Pantoprazole, I plan to do a control eradication test next week. There is no available urease breath test in my region, is a stool test a reliable alternative?

it's not good to stop the treatment midway. So try to complete your metronidazole, clarithromycin, bismuth, pantoprazole treatment. This combination looks good. Sometimes, the resistance can be overcome by having strong antibiotics combination.

I did not stop, I finished the treatment to the end. Do you think that this combination just of 7 days was enough for eradication, even if there is dual resistance? Have you encountered cases where bismuth is prescribed for 21 days? I stopped taking pantoprazole after a month of therapy due to side effects.

we only prescribe bismuth for 10 days.

if the treatment works, it works. If it doesnt, it's ok, there are many options.

Thank You for your prompt response. Is it possible that these ongoing symptoms are due to gastritis and damage done by helicobacter pylori? There are not as severe as they were before treatment. If this first line therapy fails, do You think that combination of pantoprazole, bismuth, rifabutin and ciprofloxacin could work for me? Thank You again.

It is possible that symptoms are due to the inflammation caused by H. pylori. So it is important to have it eradicated to reduce inflammation.

it is also possible that H. pylori can cause some permanent damage and so symptoms may persist. But that usually happened among the elderlies.

Nevertheless, pantoprazole + bismuth + rifabutin + ciprofloxacin are good rescue therapy if you are still positive of H. pylori.

Ok, thank You. Next week I'm going to do a stool test, it's 6 weeks since I'm off bismuth and 2 months off antibiotics, hope that's enough period for retest. I'll keep You updated.

P. S. Do I need to stop with natural remedies, like herbal teas, cranberries, and laxatives before retesting?

if your remedy is going to affect your stomach acid, then stop. otherwise it is ok.

My stool test came back positive today. Should I pause before my second line therapy, and check in the medical history previous antibiotics usage? What therapy do You suggest, since my previous doctor seems unconcerned? I'm considering to make a break, because I'm afraid that second line of antibiotics would destroy my gut and immune system, and I wouldn’t want that to happen while the corona virus and seasonal flu pandemic is ongoing.

It's up to you if you want to get another treatment immediately. We normally just prescribe the second treatment straight away.

Rabeprazole 20mg TID
bismuth +
rifabutin +
ciprofloxacin

is a good combination. Ask your doctor if you can have this combination.
