Hi Helico,

I have been having bloating, continuous burping, fullness upper abdomen and burning chest sometimes. I did endoscopy a week ago and doctor diagnosed Mild Gastritis. He said it could be because of health anxiety and stress related. I do have sedentary lifestyle as I am working from home and I do have health anxiety, but I did not know I would end up messing up my stomach. I am on Nexpro, a combination of DOMPERIDONE-30MG + ESOMEPRAZOLE-40MG twice a day and I have changed my diet to completely blan and boil. Eating veggies and fruits. However symptoms are pretty much the same

My question is that doctor did not consider testing for autoimmune disease or H Pylori. He said H Pylori bacteria are not seen in endoscopy and hence no test required. if that's the case then why do I have inflammation in stomach. Should I insist on getting tested for H Pylori? What is causing these symptoms if the case is Mild and why it has not been improved?

P.S I had severe case of acute gastritis 8 months ago and after my VIT B12 dropped to 112 pg/ml. I took injections and got it to 400 pg/mL. Is this a case of Pernicious Anemia?

getting a test of H. pylori is easy and should get it done.
I am not sure how did your gastroenterologist diagnose H. pylori during endoscopy. Perhaps via CLOtest. it can gives false negative if you had been taking PPI.
What about histology? did your doctor order one? it is more accurate for detecting H. pylori.

otherwise, a breath test or stool test can be easily done to detect H. pylori. Then again, they both can be affected by PPI. So you will need to stop PPI for at least 4 weeks before doing the test.

Thanks for your response. The doctor did not do a biopsy nor a histology. He just performed an endoscopy and declared that my gastritis is due to stress and nothing else. With H Pylori the inflammation would have been more severe he says. So he didn't feel the need. I am thinking to approach a path lab and do the H Pylori test myself. I wanted to know which is more accurate in terms of detecting the HP, is it the Stool, Breathe or Blood test? As you mentioned I have to off PPI atleast for 4 weeks prior to the test. Unfortunately this can only happen once my course of the treatment is complete. I will wait and see

Meanwhile, I would like to ask the following

1. Does Mild Gastritis/ Gastritis takes longer time to heal even if its not caused by H Pylori/ Autoimmune condition?
2. Can it progress to something more severe in long run like IM or Gastric Cancer?
3. What is stress related gastritis? Does it really even exists or it is something cooked up by Doctor when they don't understand the root cause of the problem

H. pylori is usually the reason for the stomach inflammation. However, in very very rare condition, the inflammation can be caused by autoimmune.

depending on the severity of the gastritis.. some very severe inflammation can lead to IM or gastric cancer.

stress will not cause inflammation (or gastritis). However, stress can contribute to gastric symptoms similar to gastritis. Just like when you are facing deadline, or nervous, or before exam period, you can sometimes feel gastric symptoms.

Lastly, stool test and breath test are equally good in detecting current infection. But you need to stop PPI for at least 1 month before doing the test. You can also switch to H2 blockers which will not affect the stool or breath test. You only need to stop H2 blocker one day before the test.

Thank you for your valuable feedback. If the inflammation is caused by H Pylori then why the doctors whom I have consulted are not prescribing the H Pylori test. Why do they think the PPI and lifestyle will resolve the problem. I am very confused.

Have you come across any case where H Pylori associated stomach inflammation resolved on its own?

hmmm... I am not too sure how your doctor diagnosed that you are free of H. pylori. Nevertheless, you can always get a breath test to check if you are actually infected with H. pylori.

Hi, Just to reiterate my condition. I had an endoscopy on 30th December 2021 and the doctor mentioned it is 'Mild Gastritis', he said it could be due to stress related. Unfortunately he did not do biopsy for H Pylori. He put me on Nexpro for 20 days, a combination of DOMPERIDONE-30MG + ESOMEPRAZOLE-40MG twice a day. I have changed my diet to completely bland and boil. Eating veggies and fruits. Today is the 20th days my condition is pretty much the same and If I don't take PPI my condition is aggravated.

Today I have visited the GASTRO and he says the condition should have resolved by now, evidently it didn't. So he prescribed below HP kit for 7 days,

Esomac Hp Combo Combipack Tablet is a combination of three medicines: Clarithromycin, Esomeprazole, and Amoxicillin

Please advice if these antibiotics and the duration of 7 days is the right course of treatment for H Pylori

Hi,

The endoscopy looks normal. At least there isnt any serious abnormality detected.

have you started your HP7 treatment? Have you had it before?
