My question is what is the role of fatty foods in symptom flare up? My understanding is h pylori cannot survive acidic environment, so why people get their symptoms worsened after eating fat based meals. Is it due to excess acid killing off bacteria which in turn irritate stomach?

when you eat very fatty or oily food, your bile can become very active. So perhaps your symptoms is due to your bile.

Although H. pylori doesnt like acid, it has its hiding place in the stomach where there is no acid.
