Cefixime 200mg twice a day + 20mg omeprazole twice a day for suspected H pylori

The most popular treatments consist of two antibiotics (one of which is usually clarithromycin) plus a strong acid blocker such as omeprazole (PriLosec, Nexium, Somac, Zoton, etc.). Treatment is for 7-10 days. Cure rate is about 85%

Cefixime 200mg twice a day + 20mg omeprazole twice a day for suspected H pylori

Post by Myrad56 »

Just wanted to know the efficacy of this treatment for suspected new H pylori infection with Cefixime 200mg twice a day + omeprazole 20mg twice a day,
can it be effective in inhibiting or slowing the growth of H pylori until I get the diagnosis done? Since I have been avoiding to go anywhere even near a hospital or a medical facility due to intense covid19 spread around my area. My symptoms are just mild burning sometimes and somedays black tarry stools.

I was actually taking this combo for mild UTI but read online in some nih research papers that cefixime has the highest activity against H Pylori among many cephalosporins.

with only Cexime and Omeprazole, the cure rate is probably around 50% or lower.

you need a very strong dose of PPI to make this treatment work against H. pylori.

good question!
you might have to try vonoprazan, a new type of PPI. Not sure if that's available where you are.

otherwise, you might have to try rabeprazole, 20mg TID.

So Rabeprazole 20mg TID + cefixime 200mg twice a day for how many days ? I have completed my 6th day with cefixime 200mg BID and omeprazole 20mg BID

you almost complete your treatment. I am not sure how much will it help if you change your PPI now. I wonder where your doctor get the idea of having Cefixime dual drug therapy. Cefixime is not commonly used for H. pylori. Is there a reason why?

Like I posted above, its actually prescribed for UTI/Prostatitis and for which the dosage was for 14 days.

So I wondered whether it could wipe out two infections at once or at least inhibit the growth of H Pylori upto some extent along with the ongoing UTI treatment ? shuffling PPIs won't be much hassle if it could help eradicate H Pylori

Should I keep taking the same PPI ?

One update is that the frequency and intensity of the burning pain in belly has gone down but maybe that could also be because I have been taking either Ranitidine or omeprazole interchangeably for the last 2-3 months at least but started taking the regular dosage with cefixime only now in the last week. I know we shouldn't take omeprazole for more than 14 days in a row but it gives me the best relief.

Sometimes even with Omeprazole the mild burning sensation comes after meals then I top up with a single dose of Ranitidine 150mg to get some relief.

you can continue omeprazole or ranitidine or both.
some people take them for years.

I think you just finish the current course of treatment and then see how it goes
